Hyaluronic acid wrinkle fillers are an intervention to correct facial volume deficits.

Hyaluronic acid (HA) is found in many places in the mammalian body (skin, vitreous humour of the eye, synovial fluid, cartilage, bone, heart valve, umbilical cord). Its water-binding capacity is 1,000 times its own weight. HA does not show species or tissue specificity and therefore the likelihood of a hypersensitivity reaction is minimal. When injected into the skin, natural HA degrades within 24 hours and is cross-linked to achieve a longer duration of action. In addition to its immediate volumizing effect, HA also has collagen regenerating properties. There is also an antidote, the enzyme hyaluronidase, which breaks down the unwanted filler (in case of bee or wasp venom allergy, hyaluronidase injections may be contraindicated due to the possibility of cross-reactivity).

Depending on the patient's sensitivity, a local anaesthetic cream can be applied before the procedure, and 20-30 minutes are needed to achieve the desired effect. Juvederm® fillers contain lidocaine to reduce discomfort. After skin disinfection, the filler is injected with a thin needle into the skin or subcutaneous layer to be treated. The treatment takes 20-40 minutes. The final result is achieved in 2 weeks. The durability of the result depends on the filler chosen, the injection technique, the skin texture, age and lifestyle of the patient. Avoid sunbathing, tanning beds, saunas for 2 weeks after the treatment.

Treatment is contraindicated in case of hypersensitivity to any of the fillers, existing infection of the skin area to be treated, presence of wound, intestinal infection, hives, active autoimmune disease, increased haemophilia (due to illness or medication). Medication is required to prevent herpes virus activation.

Undesirable side effects may occur as a consequence of individual reactions, anatomical abnormalities, underlying medical conditions, medications taken, the filler or the injection technique. The complication may be immediate (within 24 hours), early (within 4 weeks), late (beyond 4 weeks). Frequent and transient pain accompanying the procedure, severe pain may be a sign of vascular complication. The face is richly vascularised, so haemorrhage is common, but it is also predisposed to hypertension, low platelet count, coagulation disorders, vascular diseases, blood-thinning drugs, non-steroidal anti-inflammatory drugs (NSAIDs), fish oil, vitamin E, ginseng, Gingko biloba.

It is advisable to avoid bowing and vigorous exercise for 24 hours after the procedure. In the event of haemorrhage, it is recommended to cool the area, apply pressure and apply a cream containing vitamin K, arnica or black celandine. A pigment spot may also form after haemorrhage or inflammation. Trauma from needle sticks may also cause oozing, which is often temporary and may be helped by cooling, or in more prolonged cases by ibuprofen or other NSAIDs. Hypersensitivity reactions are rare and can be immediate or delayed, with the formation of nerve body granulomas (nodules). Eczema, diabetes, immunosuppressed state predispose to infection. Avoidance of facial contact in the hours following the charge, no make-up for 24 hours. In the days following the filling, it is recommended to avoid very strong mimicry, facial massage, strenuous physical activity with facial movements and shaking to prevent displacement of the material. Temporary nerve damage may be caused by compression of the needle or filler, resulting in loss of sensation for a few weeks. Rarely, but the most serious is vascular complications, where the filler enters the vessel and blocks it or compresses the vessel from the outside. A vascular occlusion can lead to anaemia, necrosis or necrosis of the area supplied by the vessel, and a rare complication is loss of vision. In the case of venous occlusion, reddish-bluish discoloration, blisters, necrosis, scarring may occur. If hyaluronidase is injected immediately, permanent damage can be prevented.
